What are the required forms?

For every camper, we need the Hold Harmless and the Medical Form completed 30 days before the start of camp, or as soon as you register if within 30 days. Please note that per the Medical Form, required attachments for submission also include a recent (within one year) physical and immunization records for the registered camper. Additionally, if your camper will be taking any medications during camp programming, then their physician must fill out the information in the box on the Medical Form

When are forms due?

Forms are due 30 days prior to the start of camp. If registering within 30 days of the start of camp, we ask that they be submitted as soon as possible. Please submit forms prior to check-in so the medical director has an opportunity to review them.

How do I fill out and submit my forms?

The Hold Harmless Agreement requires a parent/guardian signature. The Medical Form must be completed and submitted with up-to-date (within one year) physical/immunization records. If your child takes any medications then their physician must fill out the information in the box on the Medical Form. The physical must be current through the end of the camp.

How do I know my forms have been received?

Please allow 2-3 weeks for your forms to arrive and be processed. We will send you an email upon receipt and subsequent processing of the forms. The status will be available online with your user ID and password (created by you at time of registration). Please note that a green check mark next to the associated form within your registration account denotes successful submission and processing.

What happens if I do not send my forms in before the start of camp?

We appreciate receiving forms prior to the start of camp as it gives our Medical Director the chance to review the health status of all participants, note conditions that require particular attention, and ensure all campers are eligible to participate. If this is not feasible, we ask all forms forms are brought to camp check-in completed to ensure a smooth check-in experience for all campers. The Medical Director will review all new submissions on-the-spot and may require additional information to be provided.
