No, it is not necessary for your camper's physician to sign the Cornell form, as long as you can submit the completed form along with a recent physical signed by the doctor. The physical must have been within a year of the last day of camp. Please note, a physicians signature is required if your camper is required to take medications while attending camp (information in the box on the Medical Form).
