What are the required forms?
Every multi-day camp has at least three required forms. They include a Hold Harmless Agreement, a Rules and Regulation Form and a Medical Form.
If you are staying on campus for more than seven (7) consecutive days we require a signed meningitis waiver. There is an additional required waiver for all participants in Rock Climbing camps that must be turned in at the time of the other required forms.
When are forms due?
Forms are due 30 days prior to the start of camp. Late payment and/or late submission of the required forms will result in a $20 late fee.
How do I fill out and submit my forms?
Forms are available here (or from the website menu). The Hold Harmless Agreement requires an insurance ID number and a parent/guardian signature. The Rules and Regulations needs to be read over and signed by parent/guardian and child. The medical form must be filled out and signed by a doctor. We will also accept an up-to-date physical from the previous year showing the child to be in good health. The physical must be current through the end of the camp.

How do I know my forms have been received?
Please allow 2-3 weeks for your forms to arrive and be processed. We will send you an email upon receipt and subsequent processing of the forms. The status will be available online with your user ID and password (created by you at time of registration).
